How do avoid duplicate action assignments in form chaining?

Hi, we have a follow up process where we split the User Entry Form into 2 parts.

We email the first to give the assignee a button to access details of the person they have been asked to contact.

When they have made contact and want to give feedback, they click 'give feedback' and enter a comment.

the issue is 2 duplicate actions are generated in the My Tasks lava detail block?

Can anyone assist please?

If the assigned activity is running twice, you may need to mark the previous one complete somewhere.
Can you post the outline of your activities and actions here so we can start to see where the issue might be?
