Project Plan template for migration from FellowshipOne (or any ChMS) to RockRMS

Does anyone have a project plan that they've used to help them migrate from an existing ChMS software to RockRMS?  I'm open to creating one for our project, but no sense reinventing the wheel if there is already something that can be leveraged.  Any thoughts/assistance would be appreciated.
