Our Youth Director wanted a way for parents to complete a form to give information about their student's extracurricular activities. The intention was to inform our Youth staff and volunteers so they would be able to attend the different student events. In order for this information to be useful, they asked if the form could automate to a visual calendar so they could see the upcoming student events in one place. They also asked if there was a way to visually indicate which events someone had committed to attend versus the events that no one had committed to.
