Follow event on selectable date attribute

I'm looking for a way to receive follow notifications based on a unique person attribute. I did some digging to build this myself but it's out of my depth. 


It appears that the baptism follow notification is based on the attribute key. I would love to be able to have that attribute key stripped and enter whatever attribute key and description I would like.
